Please bring your insurance card and Photo ID to your appointment. The office will also need a full list of your medications, your local pharmacy info and your referring or primary doctor information. If you are transferring your care from another physician to our practice, please have your medical records faxed to our office prior to your visit. This will prevent any confusion regarding previous treatments.

During office hours, please call (407) 389-2020 and our office staff will be happy to assist you. During non-business hours, if you have a true medical emergency then please call 911. If you need to reach Dr. Witt, Laurie or Nikki after hours, please call the office at (407) 389-2020 and follow the appropriate prompts.
For your convenience, handicapped parking is available to our patients in the parking lot behind the office building and patients can enter through the rear office entrance (just push the doorbell when you arrive).
Our new office is located next door to Park Maitland School and we want patients to be aware that every day between 7:30am-7:45am and 3:20pm-3:45pm, there will be traffic as you approach our office from either direction. This traffic can cause a 12-15 minute delay. We will minimize the number of appointments at these times of day to avoid any inconvenience to our patients, but we ask that you allow extra time to get to our office at these drop-off and pick-up times.
We accept most insurance plans. All patients with HMO plans should check with their insurance company to verify if authorization is required before coming in for an appointment.
