When calling or requesting an appointment online, please have your insurance information readily available. It will be required for you to activate the patient portal to access your records, appointments, request prescriptions and to view or update your medical history.
Please have your insurance card available when scheduling and checking in for an appointment. If a referral is required, it is mandatory that you obtain one from your primary care physician and bring it with you on the day of your appointment. Otherwise, we may need to reschedule your appointment if one is not provided.
We participate with most insurance plans and as a courtesy, we will verify your benefits and submit claims to your insurance carrier. Not all services or procedures may be covered by your insurance policy. Our office will do our best to give you an estimate of what charges may be your responsibility at the time of service. If you have an office co-payment, deductible or co-insurance, please be prepared to pay it at the time of your appointment.

Returned Checks are subject to a fee of $25.00 and will be added to your account as a balance due to the practice.
If you are a Self-Pay patient because we no longer accept your insurance plan or you find yourself without any health insurance coverage, our providers are still happy to see you and will work with you to achieve an affordable treatment plan. Self-Pay patients are required to pay at the time of service and we will always give you an estimate of your charges before any procedures are performed. Please see our self-pay prices here.
If you arrive late for your appointment, you may be asked to reschedule or see another provider that may have more availability. All patients are seen by appointment only. We ask that you contact us 24 hours in advance to cancel or reschedule your appointment so that we can offer that slot to another patient. Please note that without giving proper notice, you may be assessed a fee for not showing to your appointment or cancelling your appointment within the same day.
Please bring your insurance card and Photo ID to your appointment. The office will also need a full list of your medications, your local pharmacy info and your referring or primary doctor information. If you are transferring your care from another physician to our practice, please have your medical records faxed to our office prior to your visit. This will prevent any confusion regarding previous treatments.
A parent or legal guardian must attend the initial appointment with the child and all appointments involving invasive treatments. Minor children must be accompanied by a parent or authorized individual age 18 or older for all appointments. In the event a parent cannot bring the child to their follow up appointments, a parent or legal guardian can execute the Parental Temporary Assignment of Authorization to Treat Minor Child. Please note, the signature must be witnessed by a staff member of Derrow Dermatology.
Prescriptions will be filled during business hours. The prescriptions are usually submitted to the pharmacy by the end of the day of your visit. For prescription refills, please leave a message for our medical assistants and they will return your call by the end of the following business day. You may also reach us through our patient portal and write the provider or nurse a message for a refill request. All patients who have had a recent biopsy or surgery will be contacted with their results within two weeks of the procedure. If a patient has not heard from us after two weeks, please call the office or contact us through the patient portal to request your results.
We accept most insurance plans. All patients with HMO plans should check with their insurance company to verify if authorization is required before coming in for an appointment.
