To better serve our patients, we would like to invite you to access the Patient Portal which can be found at dda.ema.md.
When your appointment has been scheduled, a link will be emailed to you and you will be prompted to sign on to the patient portal to create a password. The link expires 72 hours from when the initial email has been sent. If you are unable to activate the portal within that time, you can call into the office for a reset or wait to arrive at your appointment and complete the process at check-in. Of course, this can take some time, so we require that you arrive 15 minutes prior to the appointment to complete this process.
Once on the portal, we will need you to complete the following information as detailed as possible to assure the registration process for your appointment is prompt and efficient.
- Add, review or change your contact information.
- Add or make changes to your medical history.
- Add or make changes to your medications and allergies.
On the patient portal, you can also
- Communicate with your provider and/or their staff.
- Request prescription refills.
